Regulation on the College for Quality Certificates

With further information about objective and composition of the College for Quality Certificates


1. Place
The College of Quality Certificates is set up by the Board.


2. Objective
The College of Quality Certificates advises the NIAZ Board, or rather regarding a positive recommendation, takes a mandated decision regarding granting or continuing the accreditation status of a health care organisation.


3. Composition of College and appointing the members
3.1 Composition
The College of Quality Certificates consists of a chairperson, vice chairperson and at least four members.

Also participating:
 the Director of the NIAZ
 the coördinator of accreditation programmes / Head of the NIAZ office.

3.2. Appointing the chairperson and members of the College of Quality Certificates (CQC)
 The chairperson and vice chairperson are appointed annually by the Board.
 Members are recruited from the pool of auditors.
 Members are nominated by the NIAZ Directorship.
 Members are appointed by the Board.

Members are appointed for a period of two years with the possibility of being reappointed once. They resign according to a resignation timetable. Reappointment is possible after a period of withdrawal.

4. Decision-making process
The recommendations of the College to the Board, regarding granting or continuing the accreditation status, are based on the consensus of the college members and chairperson. The following recommendations can be distinguished:

- A positive recommendation
The CQC takes a mandated Board decision about granting or continuing the accreditation status.

- A negative recommendation
The CQC reports this recommendation, substantiated in a report, to the Board. The Board takes a decision.

- Postponing a decision
The CQC reports this recommendation, substantiated in a report, to the Board. The Board takes a decision.

Decisions are only made when, in addition to the chairperson, at least four members are present. Should a member of the CQC have been involved in an audit, then he/she cannot participate in the CQC handling of it.

When no consensus is reached regarding making a recommendation, the director will draw up a report for the NIAZ Board. The Board will make a decision on the basis of the report.


5. Reporting
A report is made for each audit and the recommendation brought out about it. The report is signed by the chairperson.

6. Confidentiality
The chairperson and members of the College sign a Declaration of Confidentiality.

7. Reimbursement
The chairperson and members receive a reimbursement annually as established by the Board.

8. Other matters
Other matters are regulated in the ‘Procedure on the College of Quality Certificates'.
