Publication Policy

According to Article 4 of the standard accreditation agreement NIAZ decides on the information to be published regarding healthcare organisations that participate in the accreditation program. Below is listed the current regulation.

Article 4 of the standard accreditation agreement

paragraph 1 The executive board of the NIAZ establishes the publication policy in relation to the accreditation status and the accreditation process of the care institutions affiliated to the NIAZ. The publication of the institution’s data will take place in a comparable manner to that of a comparable care institution.

paragraph 2 With due regard to the publication policy, the NIAZ will observe confidentiality in relation to all that it learns about the institution in the context of the accreditation process. This concerns all the information which is not accessible to the public or third parties in a legitimate way. For this purpose, the NIAZ has all persons affiliated with the NIAZ, no matter what title they may hold, sign a declaration of confidentiality, which will remain in effect after the affiliation with the NIAZ has ended.

paragraph 3 The NIAZ will archive information about the institution properly, and will ensure that it is not accessible for unauthorized persons and that it is destroyed adequately should the case arise.

 Publication Policy

Scope
1. This publication policy concerns, subject to that stated in article 3, the care institutions with which the NIAZ has or has had an accreditation agreement.

Publication medium
2. The NIAZ publishes information about the accreditation status and the accreditation process of the institutions affiliated with the NIAZ via its website (http://en.niaz.nl/).

Pre-reserving
3. Institutions which have not yet achieved accreditation status but are preparing for it, may request to be mentioned on the NIAZ website, with the note that they are in preparation. In this case, an estimate of the date that the accreditation agreement will commence and the date of the audit visit will be noted.

First accreditation agreement
4. When a first accreditation agreement has been reached, this will be immediately published on the website. The website mentions the following for each named institution:
a. the date that the accreditation agreement commences;
b. the expected date of the audit visit;
c. the expected date for the accreditation decision.

Accreditation and Re-accreditation
5. In the case that an institution is accredited or re-accredited the website will mention the following for each named institution concerning the actual accreditation status:
a. the date that the accreditation agreement commences and its nature;
b. the date of the application for accreditation or re-accreditation;
c. the date of the most recent audit visit and, if applicable, the date of the assessment of the action plan;
d. the date of the accreditation decision;
e. the accreditation status;
f. the period for which this has been granted;
g. an estimation of the date of the next re-application and the next audit visit.
Publication will take place after the institution in question has been informed in writing and once the period for lodging an appeal according to the Regulation Complaints and Appeal has expired. Should no decision have been made which is restrictive or subject to appeal for the institution, then publication will take place immediately.

Deferment
6. In the case that the accreditation status for an institution is deferred by the NIAZ, the website will mention the following in addition to that mentioned under 2:
a. the date for the deferment decision;
b. the grounds on which the deferment decision is based;
c. the period of time in which the institution is given the opportunity of reversing the deferment.
Publication takes place days after the institution has been informed of the deferment by registered post.

No accreditation or re-accreditation
7. In the case that an institution is assessed by the NIAZ and is not accredited or re-accredited, the website will indicate the following for each named institution concerning the actual accreditation status:
a. the date that the accreditation agreement commences and its nature;
b. the date of the application for accreditation or re-accreditation;
c. the date of the most recent audit visit;
d. the date of the decision to not grant accreditation;
e. the grounds of the evaluation for not ascribing accreditation status;
f. the reasons why the accreditation status has not or not yet been ascribed or not or not yet been renewed;
g. the period of time after which the institution can submit a new application, as well as any possible conditions attached to this.
Publication takes place after the institution in question has been informed in writing and the period for lodging an appeal according to the Regulation Complaints and Appeal has expired.

Publication in the case of appeal
8. - Lodging an appeal does not defer the publication of the disputed decision on the grounds of any of the preceding articles, unless he operation of the decision in question is deferred on the grounds of the accreditation agreement or pursuant to a decision by the executive board;
- Should publication not be deferred, the fact that an appeal has been lodged will also be indicated on the website, if possible with an indication of the grounds which were put forward;
- If the appeal is declared well-founded, the data published on the website will be altered to the situation as it would have been if the NIAZ had already reached this decision in the first instance.

Historical file (track record)
9. For each named institution, in addition to the data as indicated above, the website indicates a historical file concerning the actual accreditation status, in which all the official applications made by the institution and all the decisions taken by the NIAZ regarding the accreditation status are documented. One thing and another with due regard to the last sentence of article 8.

Information after withdrawing participation in an accreditation programme
10. Should an institution no longer participate in the NIAZ’s accreditation programme, the historical file will remain accessible on the NIAZ website. The website indicates the nature of the institution’s withdrawal from participating in the accreditation programme.
